Sundara Purushan

A struggling musician takes a job at a mental hospital, owned by a woman who becomes violent when he refuses to sing, revealing her own mental illness, and as part of her treatment, he pretends to love her before eventually fleeing to his hometown to see his wife and daughter, but is convinced to marry the woman as an act, leading to conflict with his wife until the womans mental state improves, and the story ends with a twist involving divorce papers and a realization of love.
